I don't see any 2.54mm pitch SM components in the library. (At least not in the indexes.) Don't you use these? Or am I blind?
These are common for optoisolators. The manufacturer just modifies a standard .1" pitch DIP package by snipping the leads short and bending them out to make tiny feet.
Digikey calls these out as 6-SOP packages, but maybe you call them something else...
