PCB Manufacturer specification form

Does any one have a form that would be given to a PCB Manufacturer to have them fill out to specify the make up of the PCB they are suppling. Seams moreoften than not I need to drag the info out of them.

EX. PCB material type, suppplier and board material number
Silkscreen type, material suppplier and material number
Soldermask type, material suppplier and material number
Solder type, material suppplier and material number

etcetera....

Of course most fab houses will supply a Certificate of Comformance which will list the pcb material and supplier.

I have never seen the rest, but I suppose a form could make sense. Seems it would be simple enough to make one with those items you have listed.
